SAU 24 Wellness Initiative

Employees enrolled in the Lumenos High Deductible health plan are offered incentives to maintain a healthy lifestyle. It is possible to earn up to $1,000 annually to be contributed to the employee Health Savings Account. Activities with incentives include:

Program Incentives

From HealthTrust

SmartShopper: If you are planning on having a procedure - from basic blood tests as part of your annual physical to surgery - Shop for services and receive incentive money based on your preferred provider. Click on "SmartShopper" then "Shop for Service". Different providers will be listed with incentive amounts. If you use one of these preferred providers, you will receive a check in the amount shown. 

Online Health:  Complete an annual Health Assessment then track points and earn incentives for activities like talking with a health coach, tracking physical activity, getting a biometric screening, and taking wellness classes. Get details by clicking Online Health then "earn points"

Forms: Any forms you need can be found by clicking on the Forms menu choice.

From Anthem

Each year you will be asked to submit a summary of your claims. Log into the Anthem site, click "Claims" then "Deductible & Out of Pocket  Amounts". Print this screen, write your full name on the top, and submit as requested.

Program Update – Now You Can Earn More Than Ever Before In 2017,

HealthTrust's Slice of Life participants can earn up to $475 in rewards – the most ever offered by the program. 

• You can now receive $50 for completing a Health Assessment and $75 for completing a Biometric Health Screening. (These reward amounts have been reversed from previous years.)

• You can now receive $50 (and up to 200 points) per year for engaging a Health Coach (the first time a cash reward has been offered for this activity).

• By connecting a mobile fitness device and/or using online trackers you can earn up to 2,600 points toward cash rewards in 2017 in the Points Program (compared to 2,400 in 2016).

• You now have more opportunities to win valuable prizes in the four quarterly drawings. For every 250 points you accrue per quarter you earn one entry into a drawing for five $1,000 gift cards and 50 $100 vouchers for online health products through Onlife Health.
